  • Student has a grievance. Your grievance is related to an academic process or program but not an accommodation or disability. If you have attempted to go through your instructor, his/her department chair and the dean of the college, you may benefit from a conversation with the Dean of Students Office (801) 422-2731.

  • Student has a grievance. Your grievance is connected to a faculty-related issue around an accommodation or disability in general. Your grievance is related to a faculty member, but you do not want to file a formal complaint. Your grievance is related to an on-going accommodation issue, and you would like help working it out. Contact your UAC coordinator who can help you with the next steps in the process.
  • Student has a grievance. Your grievance is connected to a faculty-related issue around an accommodation or disability in general. Your grievance is related to a faculty member, but you do not want to file a formal complaint. The incident has passed, and you do not immediate action from the UA, but you would like the UAC to follow up with your instructor to make changes for future students. Contact your UAC coordinator, who can then reach out to the instructor.
  • Student has a grievance. Your grievance is connected to a faculty-related issue around an accommodation or disability in general. Your grievance is related to discriminatory actions of a faculty member, and you would like to file a formal complaint or have an official record of your complaint made. Contact Collette Blackwelder.

  • Student has a grievance. Your grievance is related to UAC staff or accommodative services provided. Your concern relates to the UAC’s role in carrying out the interactive process of an accommodation. Contact Collette Blackwelder.
